Rating from 3.00-2.50 = Exceed standars, (Pay increased 2%).
Rating from 2.50-1.50 = Meets standards (Pay increased 1.5%).
Rating from 1.50-0 = Doesn't Meet standards (No Pay increase).
* Question #5: Food safety is waited more heavely. Rate each area
(Service, Production, Food safety) and the form will do the math.
